No treatment plan will be undertaken without expressed informed consent. However, we are committed to our patients’ interests but the clinical outcome of any procedure can never be guaranteed.

We conduct our medical practice in a businesslike manner, employing all the benefits of economies of scale, technology and organization efficiency.

We are committed to quality and personalized treatment plans for our patients. We work with other health care providers to ensure a team approach to pain management.

We recognize that our continued success requires a firm investment in human and technical resources. We commit to the investment of time, energy and financial resources as an integral part of our practice so that the quality of our organization can be continually improved to the highest level. We recognize that optimal outcomes can only be achieved with mutual acceptance of accountability and responsibility.

SINW presents and educates patients and their families with responsibilities as appropriate to the services provided. Responsibilities include at least the following:

  • Providing information. The patient is responsible for providing, to the best of his or her knowledge, accurate and complete information about present complaints, medical records, past illnesses, hospitalizations, medications, and unexpected changes in the patient’s condition.
  • Asking questions. Patients are responsible for asking questions when they do not understand what they have been told or what they are expected to do.
  • Following instructions. The patient and family are responsible for following the preoperative and post discharge care plan. They should express any concerns they have about their ability to follow and comply with the proposed care plan or course of treatment, including anesthesia or operative requirements. Every effort is made to adapt the plan to the patient’s needs and limitations.
  • Accepting consequences. The patient and family are responsible for the outcomes if they do not follow the care plan.
  • Following rules and regulations. The patient and family are responsible for following the practice’s rules and regulations concerning patient care and conduct.
  • Showing respect and consideration. Patients and family are responsible for being considerate of the practice’s staff and property.
  • Meeting financial obligation agreed to with the practice.

Patients are educated about their responsibilities during their initial visit and as needed thereafter.

The patient’s family or surrogate decision-maker assumes the above responsibility for the patient if the patient has been found by his or her practitioner to be incapable of understanding these responsibilities, has been judged incompetent in accordance with law, or exhibits a communication barrier.

How do I schedule an appointment?

Telephone our office for an appointment and we can usually accommodate new patients on short notice.

What should I expect on my first visit to Spine Institute Northwest?

Your first visit will last 2-3 hours. Arrive at least 30 minutes early to allow time complete initial paperwork.

Please bring the following:

  • Your complete medical record including:
  • All previous evaluations from other providers (MDs, Chiropractors, Physical Therapists, Psychologists, etc.)
  • X-ray or MRI interpretations from recent tests (PLEASE BRING THE ACTUAL IMAGE FILMS)
  • Please bring a cash or credit card for any applicable co-payments

Physical Examination

The evaluation will mostly occur in a private, comfortable examination room. The physical assessment will be thorough and will begin by discussing your medical history, onset of the problem, and your lifestyle and work-related habits. Please also know the names of the medications you are currently taking.

The next portion of the exam will include posture and movement assessment, special tests, and palpation of specific structures related to your complaint in an effort to determine the cause of the problem. When applicable, Spine Mouse Medical device may be used in your first or second visit for digital assessment of range of motion limitations (inclinometry study).

After the examination, the physician will discuss your results with you. Further diagnosis may be necessary to determine appropriate treatment.
